Sonographic findings of the Complicated Acute Cholecystitis




Abstract
Twentyfive patients of surgically and pathologically varified acute cholecystitis with its complications were analysed as follows;
1. From 1985 to 1987, surgically confirmed acute cholecystitis were 70 cases. Among them, 25 cases showed its complication occupying 35.7% of incidence.
2. Those were 13 cases of gangrenous cholecystitis, 8 of gallbladder perforation, 2 of emphysematous cholecystits and 2 of cholecystointestinal fistula.
3. The incidence of complication was higher in old man and acalculous cholecystitis.
4. Sonographic findings suggestive of the complicated cholecystitis were pericholecystic fluid collection, abscess formation, ascites and air in gallbladder wall or lumen.Desquamated mucosa and diminished echo of adjacent liver was another helpful finding of gangrenous cholecystits.
5. Pericholecystic fluid was not a specific finding of perforation and it was also noted in ganrenous cholecystitis.
